What is Vaginal Estradiol Cream?
Oestrogen applied directly to vaginal tissue, where the effect is needed. Because absorption into the bloodstream is minimal at standard doses, it is often appropriate for women who cannot or prefer not to take systemic hormone therapy. Most people move to a twice-weekly maintenance routine after the first fortnight.

Estrogen raises the risk of cancer of the uterus lining. Estrogen with a progestin should not be used to prevent heart disease or dementia. In the Women's Health Initiative, estrogen with a progestin raised the risk of heart attack, stroke, invasive breast cancer, and blood clots in the lungs and legs. The vaginal cream carries the same boxed warning as the tablet.
Tell your care team straight away if you get any unusual vaginal bleeding, chest pain or shortness of breath, pain or swelling in a leg, sudden vision loss or change, a sudden severe headache or weakness on one side, yellowing of your skin or eyes, or swelling of your face, lips or throat.

Do not take this if you have unexplained vaginal bleeding, you have or have had breast cancer or another estrogen-dependent cancer, you have had a blood clot in a leg or lung, you have had a stroke or heart attack, you have liver disease, you have a protein C, protein S or antithrombin deficiency, or you are or may be pregnant.

How is this different from oral or patch oestrogen?
This works locally on vaginal tissue with minimal absorption into the bloodstream. It treats dryness and discomfort but does not address hot flushes or the other systemic symptoms of menopause.
How soon will I notice a difference?
Comfort often improves within two to three weeks. Tissue changes continue over several months of consistent use.
Do I need progesterone alongside it?
Generally no, because systemic absorption is minimal. Your clinician will confirm this against your history.
What should I report?
