How does Sildenafil work?

The original on-demand ED medication — taken 30–60 minutes before intimacy to support firmer, longer-lasting erections. Well-studied, well-tolerated for most men. Cardiovascular screening included in your intake.

What to expect

What to expect

  • Taken as needed — This is an on-demand tablet rather than a daily medication.
  • Timing matters — Take it 30 to 60 minutes ahead, and avoid a heavy meal beforehand.
  • Arousal still required — It supports the response rather than replacing desire.
  • Dose can be adjusted — If the effect is too strong or not enough, your clinician can change it.

Take 1/2 to 1 tablet (50-100 mg) by mouth 1 hour before sexual activity; maximum 1 dose per day

Risks & side effects

Tell your care team straight away if an erection lasts longer than 4 hours, you lose vision in one or both eyes, your hearing drops suddenly or you get ringing in your ears, or you get chest pain or dizziness or nausea during sex.

Common side effects

  • headache
  • flushing
  • indigestion
  • changes in vision
  • a stuffy nose
  • back pain
  • muscle pain
  • nausea
  • dizziness

Rare but serious

  • priapism, a painful erection that will not go down
  • sudden vision loss from reduced blood flow to the optic nerve
  • sudden hearing loss
  • a severe drop in blood pressure if you take alpha blockers
  • heart problems brought on by sex

Do not take this if you take nitrates in any form, you take riociguat or a similar drug, or you have reacted badly to sildenafil before.

This is not a complete list of side effects. Message your care team about any symptom that worries you.

Common questions

How long before it works, and how long does it last?

Sildenafil typically takes effect within 30 to 60 minutes and lasts around four to six hours. A heavy or high-fat meal beforehand can delay onset.

Do I need arousal for it to work?

Yes. Sildenafil improves blood flow in response to arousal — it does not create desire on its own.

What are the common side effects?

Headache, facial flushing, nasal congestion and indigestion are the most frequently reported, and are usually mild.

What should I avoid taking with it?

Nitrates in any form are a hard contraindication — the combination can cause a dangerous drop in blood pressure. Tell your clinician about all medications, including anything for chest pain or blood pressure.

When should I seek urgent care?

An erection lasting more than four hours needs immediate medical attention. Sudden vision or hearing changes also warrant stopping and seeking care.
