Take 1 tablet by mouth twice weekly only if estradiol elevated on labs during TRT/SERM therapy; adjust per follow-up labs

Risks & side effects

Anastrozole is FDA approved to treat breast cancer in postmenopausal women. Using it as a hormone add-on is off-label, which means the FDA has not reviewed it for this use. The side effect rates below come from breast cancer trials, so they may not describe what happens at the much lower doses used here.

Tell your care team straight away if you get chest pain or pressure, shortness of breath, a new bone fracture or sudden bone pain, severe joint pain that limits movement, or hives or swelling of your face, lips or throat.

Common questions

What does anastrozole do?

It reduces the conversion of testosterone into estrogen. On testosterone therapy, some men develop estrogen levels high enough to cause symptoms, and a low dose can bring that back into range.

Does everyone on testosterone need it?

No — most men do not. It is prescribed when labs and symptoms together suggest estrogen is genuinely too high. Suppressing estrogen unnecessarily causes its own problems.

What happens if estrogen goes too low?

Joint aches, low libido, low mood and effects on bone density. This is why the dose is deliberately small and why follow-up labs matter.

How often will I be tested?

Your clinician will usually recheck estradiol along with testosterone at your follow-up panel, then periodically after that.
