What is Methylene Blue?

At low doses methylene blue can act as an electron carrier in mitochondria, which is why it appears in longevity protocols aimed at energy metabolism and cognition. The important caveat is not the evidence but the interactions — it behaves as an MAO inhibitor, which rules it out alongside several common antidepressants.

Risks & side effects

Methylene blue can cause serotonin syndrome when taken with antidepressants and some other medicines. Tell your care team every medication and supplement you take.

Methylene blue is FDA approved as an intravenous treatment for methemoglobinemia, a blood disorder. Using it for focus, energy or aging is off-label, which means the FDA has not reviewed it for those uses.

Tell your care team straight away if you get agitation, confusion, shivering, muscle twitching or fever, especially if you take an antidepressant. Also tell us if your urine goes dark or your skin yellows, you feel unusually weak or look pale, you have a seizure, or you get a severe rash or trouble breathing.

Do not take this if you have G6PD deficiency, or you have reacted badly to methylene blue or thiazine dyes before.

Common questions

Why does it turn urine blue?

Because the compound itself is a dye. Blue or green urine is expected, harmless, and not a reason to stop.

What is the interaction risk?

Methylene blue acts as a monoamine oxidase inhibitor. Combined with SSRIs, SNRIs and other serotonergic medications it can cause serotonin syndrome, which is serious. A full medication review comes before any prescription.

What is the evidence?

Mechanistic work is interesting and clinical use at higher doses is long established for other purposes. Evidence for cognitive or longevity benefit in healthy adults is early.

Who should not take it?

Anyone on serotonergic medication, anyone with G6PD deficiency, and anyone pregnant.
