What is Metformin ER?

Extended-release metformin sensitizes insulin action and supports metabolic health — increasingly used in longevity protocols for members with insulin resistance or pre-diabetes flagged on their Vitals Vault labs.

What to expect

What to expect

  • Take it with food — The evening meal is the usual timing and reduces stomach upset.
  • Digestive adjustment — Any upset is usually early and settles as your body adapts.
  • Bloodwork matters — Kidney function and B12 are worth checking periodically on long-term use.
  • Quiet, not dramatic — This works on metabolic markers rather than producing changes you feel day to day.

Risks & side effects

Metformin can cause a rare build up of lactic acid in the blood, which has been fatal. The risk rises if your kidneys are not working well, if you drink heavily, or if you become dehydrated.

Tell your care team straight away if you feel generally unwell with muscle aches, you breathe fast or have trouble breathing, you are unusually sleepy, you get stomach pain with nausea or vomiting, or you feel cold, dizzy or very weak.

Common side effects

  • diarrhea
  • nausea
  • vomiting
  • abdominal pain
  • indigestion
  • gas
  • weakness
  • headache

Rare but serious

  • lactic acid building up in the blood
  • vitamin B12 deficiency with long term use
  • low blood sugar if combined with insulin or a sulfonylurea

Do not take this if your kidney function is severely reduced, you have a build up of acid in the blood, or you have reacted badly to metformin before.

This is not a complete list of side effects. Message your care team about any symptom that worries you.

Common questions

What is metformin used for here?

Metformin improves insulin sensitivity and reduces glucose production by the liver. Beyond diabetes care it is used for metabolic support, and is studied for healthy-ageing applications, though that use is not FDA-approved.

Why the extended-release form?

Extended-release metformin is generally better tolerated than immediate-release, with less stomach upset. It is taken once daily, usually with the evening meal.

What are the common side effects?

Digestive upset — nausea, loose stools, bloating — most often in the first weeks and often improved by taking it with food. Long-term use can lower vitamin B12, which is worth monitoring.

Who should not take it?

It is not appropriate with significantly reduced kidney function or certain liver conditions. Your clinician will review your labs and history.
