What is Liothyronine (T3)?

How to dose

Take 1 tablet (5 mcg) by mouth daily; may titrate to 5 mcg twice daily based on follow-up labs

Risks & side effects

Thyroid hormone must not be used for weight loss. Doses larger than your body needs can be dangerous, especially alongside other weight loss medication.

Tell your care team straight away if you get chest pain, a pounding or racing or irregular heartbeat, shortness of breath, tremor or sweating or feeling overheated, or you lose weight without meaning to and cannot sleep.

Rare but serious

  • heart rhythm problems, particularly if you are older or have heart disease

  • loss of bone mineral density with long term over-replacement
  • loss of blood sugar control if you have diabetes

Do not take this if you have adrenal insufficiency that has not been treated first.

Common questions

When is T3 added?

Usually when symptoms persist despite T4 therapy and free T3 remains low. It is not a first-line replacement — T4 is.

How is it dosed?

T3 is short-acting, so it is often split into two smaller doses across the day rather than taken once. Your clinician will set the schedule.

What should I watch for?

Signs of too much: a racing or irregular heartbeat, tremor, anxiety, difficulty sleeping and heat intolerance. Report these rather than adjusting the dose yourself.

How is it monitored?

Through free T3 alongside TSH and free T4. Because T3 is short-acting, timing of your blood draw relative to your dose matters — follow the instruction you are given.
